    Complete this form by July 28, 2022.  Sorry, no dingys.   Contact Jeff Slayer with any questions: 616-836-2888.  The following information will be used for live broadcast commentary during this parade, in newspaper and radio releases, and to contact you following the parade:
    A committee of judges will determine the winning boats.  Each Class will have a 1st, 2nd, and 3rd place winner with cash prizes of $100, $75 and $50.  There will also be a boater's choice prize of $50.  Sorry, no dinghys. 
    Liability Release and Waiver:   I/We, hereby agree to hold harmless and release from any and all liability and organizers, sponsors, participants, spectators or anyone directly or indirectly involved in this activity.  This Release and Waiver is for personal injury (including death) and property losses and/or damage occasioned by or in connection with this event.  I/We further agree to abide by all Coast Guard rules and regulations of this event as well as all local, state and federal laws.
